Using Health Insurance
I currently participate with the following insurance companies:

Medicare will not cover nutrition visits without a referral from your doctor for a diagnosis of DIABETES or KIDNEY DISEASE.
Please note this does not mean all insurances cover nutrition services. Therefore, you are required to call your insurance company prior to scheduling your initial visit to confirm your nutrition visits will be covered. Questions to ask.
Out of Network
If I do not currently accept your insurance, you may qualify for reimbursement. You are responsible for payment at the time of service via cash, check, or credit card. Once your balance is paid, at your request, you will be provided with a special receipt to submit to your insurance provider to request reimbursement. Most insurance companies will reimburse for medical nutrition therapy. Once you submit the special receipt to your insurance provider, the request will be processed and any possible reimbursement will be sent directly to you. All questions, or concerns that you have regarding this can best be answered by your insurance provider. Note that this special receipt does not guarantee reimbursement.
You are responsible for 100% of the service charge and any reimbursement payments will be negotiated between you and your insurance provider.
